In this full-time Primary Care Physician (PCP) role, you will deliver comprehensive outpatient care to a small panel of senior patients, with a focus on managing complex chronic conditions and improving health outcomes. You will work alongside a dedicated care team, including a medical assistant, scheduler, and referral specialist, ensuring that each patient receives coordinated, high-quality care tailored to their individual needs.

Primary Care Physician (PCP) – Summary
- Provide comprehensive outpatient primary care services to a panel of senior patients
- Focus on preventive care and chronic disease management in a value-based model
- Collaborate with a multidisciplinary care team to optimize patient outcomes
- Participate in a supportive and mission-driven healthcare organization
Duties & Responsibilities
- Deliver patient-centered, outpatient primary care services to a panel of up to 450 seniors
- Conduct thorough assessments and manage 3–5 chronic diseases per patient
- Work Monday through Friday, with clinic hours from 7:30am to 5:00pm (last patient scheduled at 4:15pm)
- See approximately 18 patients per day
- Utilize administrative time included in your schedule for documentation and care coordination
- Provide holistic care, integrating in-house consulting specialists and wellness services (e.g., cardiology, podiatry, acupuncture, tai chi, yoga, nutrition)
- Partner with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Participate in a value-based care model focused on improving quality and outcomes
- Engage in regular communication with patients’ families and caregivers
- Ensure compliance with clinical protocols and professional standards
Salary & Benefits
- Competitive base salary (starting at $250,000+)
- Incentive bonus opportunities

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Commitment to value-based care principles
- Strong interpersonal, communication, and organizational skills
- Dedication to serving a senior patient population
