In this Primary Care Physician (PCP) role, you will manage a small patient panel, allowing you to spend more time with each patient and focus on comprehensive, preventative care. Our model supports physicians with a dedicated clinical and administrative team, including a Medical Assistant, Scheduler, and Referral Specialist, ensuring that you have the resources needed to excel in your practice. The clinic offers in-house consulting specialists, such as Cardiology and Podiatry, as well as holistic health services like Acupuncture, Tai Chi, Yoga, and Nutrition counseling, promoting total well-being for your patients.

Primary Care Physician (PCP) - Summary- Full-time Primary Care Physician (PCP) position focused on senior patient care
- Physician-led, mission-driven organization emphasizing value-based care
- Small patient panel for individualized attention and high-quality outcomes
- Comprehensive support staff and in-house consulting specialists
- Provide comprehensive primary care services to a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient evaluations, preventative care, chronic disease management, and health education
- Coordinate care with in-house specialists and holistic health providers
- Utilize value-based care strategies to improve patient outcomes
- Collaborate with a dedicated care team, including Medical Assistant, Scheduler, and Referral Specialist
- Maintain detailed and accurate medical records in accordance with organizational standards
- Participate in administrative activities and team meetings as scheduled
- Uphold the mission and values of the organization in all interactions with patients, families, and colleagues
- Ensure compliance with all applicable healthcare regulations and organizational policies

- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Excellent communication, clinical, and organizational skills
- Experienced in managing senior patients with multiple chronic diseases
- Passionate about holistic, preventative, and value-based care models

- Clinic hours are Monday through Friday, 7:30 am – 5:00 pm (last patient scheduled at 4:15 pm)
- Outpatient care setting; no nights or weekends required
