Primary Care Physician (PCP) – Summary

Duties & Responsibilities
- Manage a small patient panel (maximum 450 patients), focusing on seniors with multiple chronic conditions
- Conduct 100% outpatient care with no night or weekend shifts
- Average of 18 patient visits per day
- Provide comprehensive primary care, including preventive, acute, and chronic disease management
- Work collaboratively with an assigned care team, including a medical assistant, scheduler, and referral specialist
- Participate in value-based care initiatives and quality improvement programs
- Utilize administrative time built into the schedule to complete documentation and coordinate care
- Offer patient education and support for self-management of health conditions
- Refer patients to in-house consulting specialists as needed (e.g., cardiology, podiatry)
- Contribute to a holistic approach to care, with access to wellness services such as acupuncture, tai chi, yoga, and nutrition counseling

Qualifications & Requirements

- Spanish language proficiency preferred, but not required
