Join a leading physician-led, mission-driven healthcare organization as a Primary Care Physician (PCP) in Philadelphia, PA. The practice is dedicated to providing affordable, high-quality, and patient-centered care, focusing on value-based outcomes for seniors with complex healthcare needs.
Responsibilities
- Deliver comprehensive primary care services to a panel of up to 450 patients, focusing primarily on seniors managing multiple chronic conditions.
- Conduct patient assessments, develop personalized care plans, and manage ongoing treatment for chronic and acute illnesses.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure seamless patient care.
- Practice 100% outpatient medicine with no required nights or weekends, allowing for a balanced work-life schedule.
- Maintain a manageable daily patient volume, typically seeing 18 patients per day.
- Participate in value-based care models aimed at improving patient outcomes and satisfaction.
- Utilize administrative time included in your schedule to complete documentation and care coordination.
- Collaborate with in-house consulting specialists, such as cardiology and podiatry, to address complex patient needs.
- Support holistic health initiatives, including programs in acupuncture, tai chi, yoga, and nutrition, which are offered to patients as part of the clinic's comprehensive care model.
- Ensure culturally competent care and communicate effectively with diverse patient populations, with Spanish language skills considered a plus.
Qualifications
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong commitment to the organization's mission of delivering high-quality, affordable care to seniors.
- Excellent interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary team.
- Spanish language proficiency is preferred, but not required.
