As a Primary Care Physician (PCP), you will manage a small patient panel, allowing for in-depth patient relationships and personalized care. The model is designed to ensure providers have the time and resources to focus on quality, not quantity. With administrative support and a care coordination team at your side, you can concentrate on delivering the best possible outcomes for a senior population with multiple chronic conditions.
This role is structured to support work/life balance, providing a Monday through Friday schedule with no nights or weekends, and includes dedicated administrative time. Primary Care Physician (PCP) - Summary
- Full-time position providing comprehensive outpatient primary care

Duties & Responsibilities
- Deliver value-based primary care to a panel of up to 450 patients, primarily seniors with multiple chronic diseases
- Conduct patient assessments, develop care plans, and manage ongoing treatment
- Coordinate with dedicated support staff, including medical assistants, schedulers, and referral specialists
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other disciplines
- Utilize holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling in patient management
- Maintain accurate and comprehensive patient records in accordance with clinic standards
- Participate in quality and performance improvement initiatives
- Ensure compliance with all regulatory and organizational policies
- Participate in patient education and community outreach activities as needed
- Work a Monday to Friday schedule with clinic hours from 7:30 AM to 5:00 PM; last patient scheduled at 4:15 PM

Salary & Benefits
- Competitive base salary starting at $250,000 or more, with incentive bonuses available
- Eligibility for partnership within 12-18 months, with no buy-in required

- 33 days paid time off annually
- Continuing Medical Education (CME) allowance of $3,500 plus one week off for CME

Qualifications & Requirements
- Residency trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Experience with value-based care models preferred
- Bilingual Spanish skills are an advantage but not required
