Primary Care Physician (PCP) - Summary- Full-time, outpatient-focused role serving a senior patient population

- Provide comprehensive primary care for a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient visits (up to 18 per day) during clinic hours, Monday through Friday
- Collaborate with a dedicated support team, including medical assistants, schedulers, and referral specialists
- Utilize in-house consulting specialists and holistic health services to enhance patient outcomes
- Participate in value-based care initiatives and focus on quality improvement
- Ensure thorough documentation and use of electronic medical records
- Engage in ongoing communication and coordination with patients, families, and interdisciplinary care teams
- Maintain compliance with organizational protocols and clinical best practices
