Primary Care Physician (PCP) - Summary
- Full-time outpatient Primary Care Physician (PCP) position

Duties & Responsibilities
- Provide comprehensive primary care to adults, with a primary focus on senior patients managing complex chronic conditions such as heart disease, diabetes, and hypertension
- Conduct thorough patient evaluations, develop treatment plans, and coordinate care with other healthcare professionals
- Promote preventive health strategies and patient education, supporting holistic wellness
- Maintain a manageable patient panel (up to 650 patients) and see an average of 10-12 patients per day to ensure quality care and relationship-building
- Participate in a value-based care model, focusing on metrics such as patient satisfaction, HEDIS compliance, and reducing hospital admissions
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists
- Participate in shared call schedule as determined by provider team size
- Document patient care accurately and in a timely manner using electronic medical records
- Stay current with clinical best practices and participate in ongoing quality improvement initiatives

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is preferred but not required
