The Primary Care Physician (PCP) role is fully outpatient, with a focus on value-based care for seniors managing multiple chronic conditions. You will work alongside consulting specialists in cardiology, podiatry, and additional holistic health services such as acupuncture, Tai Chi, yoga, and nutrition. Administrative time is built into your schedule, ensuring you have the capacity to provide the highest standard of care. Duties & Responsibilities
- Provide comprehensive primary care to a panel of senior patients with chronic conditions
- Conduct routine health assessments, manage chronic diseases, and coordinate preventive care
- Collaborate with in-house specialists including cardiology and podiatry
- Utilize holistic health services (acupuncture, Tai Chi, yoga, nutrition) as needed
- Work closely with dedicated support staff, including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives to improve patient outcomes
- Document care accurately and efficiently using electronic health records
- Engage in interdisciplinary care planning and case management
- Ensure patient access by accommodating reasonable scheduling and follow-ups
- Promote organizational mission and contribute to a culture of respect, inclusion, and quality improvement

Qualifications & Requirements
- Completed residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Demonstrated commitment to value-based, patient-centered care
- Strong interpersonal, organizational, and communication skills
- Ability to manage a small, dedicated patient panel and focus on complex senior care
- Eligible to practice medicine in the United States
