Primary Care Physician (PCP) – Summary
- Deliver comprehensive, value-based primary care services in an outpatient clinical setting.
- Manage a panel of senior patients with complex medical needs.

Duties & Responsibilities
- Provide outpatient primary care to a patient panel primarily consisting of seniors with chronic illnesses.
- Maintain a manageable caseload averaging 10-12 patients per day, with a maximum panel size of 650 patients.
- Utilize a value-based care approach focusing on quality, patient satisfaction, and regulatory compliance (including HEDIS metrics).
- Collaborate closely with a dedicated care team, including Medical Assistants, schedulers, and referral specialists.
- Participate in activities and programs designed to enhance patient wellness, such as exercise and art initiatives.
- Contribute to reducing hospitalizations and improving patient health outcomes.
- Participate in a shared call schedule, dependent on the number of providers in the market.
- Support patients with continuity of care, ensuring respect and dignity in all interactions.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not required.
- Active, unrestricted medical license and valid DEA registration.
- Proficiency in core office procedures, such as suturing, joint injections, skin biopsies, and basic wound care.
- Strong commitment to patient-centered, value-based care principles.
- Excellent interpersonal and communication skills.
