As a Primary Care Physician (PCP), you will be part of an innovative model of care designed to serve a small panel of patients, allowing for more personalized and attentive clinical management. You will work closely with a dedicated support team, including your own medical assistant, scheduler, and referral specialist, ensuring that administrative tasks are streamlined and you can focus on patient care. Our clinics provide holistic health services, in-house consulting specialists, and even door-to-door transportation for patients in need, removing barriers to care and enhancing patient outcomes. Primary Care Physician (PCP) – Summary
- Full-time position focused on primary care for seniors

Duties & Responsibilities
- Provide comprehensive primary care to a senior patient population with multiple chronic conditions
- Manage a maximum patient panel of 450 and see approximately 18 patients per day
- Work Monday to Friday, with clinic hours from 7:30am to 5:00pm (last patient scheduled at 4:15pm)
- Deliver outpatient care exclusively, with no nights or weekends required
- Collaborate with a dedicated care team (MA, scheduler, referral specialist)
- Participate in value-based care initiatives and quality improvement activities
- Utilize in-house consulting specialists, such as cardiology and podiatry, for comprehensive management
- Incorporate holistic health services (e.g., acupuncture, Tai Chi, yoga, nutrition) as part of patient care
- Ensure administrative and clinical documentation is completed accurately and efficiently
Salary & Benefits

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Valid and unrestricted medical license to practice medicine
