Primary Care Physician (PCP) About the Role
We are seeking a dedicated and mission-driven Primary Care Physician (PCP) to join our organization in Atlanta, GA. As a physician-led and values-based practice, we strive to transform healthcare delivery for the senior population by providing affordable, VIP-level care that supports better health outcomes. The Primary Care Physician will play a crucial role in delivering comprehensive, patient-centered care to seniors, many of whom experience multiple chronic health conditions. This full-time position offers an opportunity to make a meaningful impact while working within a collaborative clinical environment.
Key Responsibilities
- Provide primary care services to a panel of up to 450 patients, focusing on seniors with 3-5 chronic conditions.
- Conduct patient visits, averaging 18 patients per day, in a 100% outpatient setting.
- Work Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m., ensuring a healthy work/life balance with no nights or weekends required.
- Utilize administrative time included in your schedule to complete documentation and coordinate care effectively.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated, value-based care.
- Engage in partnership opportunities, with the potential for partnership typically within 12-18 months.
- Focus on value-based care models to improve patient outcomes and quality of life.
- Leverage in-house consulting specialists, such as cardiologists and podiatrists, to support comprehensive patient care.
- Promote holistic health services, including acupuncture, tai chi, yoga, and nutrition counseling, as part of the overall care plan.
- Ensure that transportation is available to patients who require assistance, facilitating access to care.
Required Qualifications
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, compassionate care to senior populations.
- Strong interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team.
