Primary Care Physician (PCP) - Summary
- Full-time position providing primary care to a senior patient population in Richmond, VA
- Physician-led, mission-driven organization with a focus on value-based care
- Collaborative care environment with comprehensive support staff and in-house specialists
Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient visits in an outpatient clinic setting, averaging 18 patients per day
- Participate in value-based care initiatives to improve patient outcomes and satisfaction
- Utilize a dedicated care team including a medical assistant, scheduler, and referral specialist to enhance patient care coordination
- Schedule includes Monday through Friday clinic hours, with administrative time incorporated into your day
- No night or weekend shifts required
- Engage in partnership opportunities with the organization, typically available within 12-18 months
- Collaborate with in-house consulting specialists such as cardiology and podiatry as needed
- Support holistic health initiatives, including acupuncture, Tai Chi, yoga, and nutrition services offered to patients

Qualifications & Requirements
- MD or DO degree from an accredited medical school
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Commitment to delivering high-quality, value-based care to senior patients
- Strong communication, collaboration, and organizational skills
