Primary Care Physician (PCP) - Summary
- Full-time Primary Care Physician (PCP) position focused on value-based care for seniors

Duties & Responsibilities
- Provide comprehensive primary care to a panel of senior patients, typically managing 3-5 chronic diseases per patient
- Conduct patient visits (average 18 patients per day) in a 100% outpatient environment
- Maintain and foster long-term patient-provider relationships
- Collaborate with in-house specialists, including cardiology and podiatry, for coordinated patient care
- Participate in a value-based care model, focusing on preventive care and positive health outcomes
- Utilize dedicated support staff, including a medical assistant, scheduler, and referral specialist
- Incorporate holistic health services into patient plans, such as nutrition counseling and wellness activities
- Attend to administrative responsibilities during scheduled time, ensuring thorough documentation and compliance
- Engage in professional development and partnership opportunities within the organization

Qualifications & Requirements
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM)
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Strong clinical, communication, and organizational skills
- Ability to work collaboratively with multidisciplinary clinical teams
- Dedication to serving senior patient populations
