Primary Care Physician (PCP) – Summary
- Serve as a Primary Care Physician (PCP) in a multi-site FQHC setting.
- Deliver comprehensive family medicine and internal medicine services to a diverse patient population.
- Collaborate with a multidisciplinary team, including behavioral health providers, social workers, and care coordinators.
- Participate in preventive health initiatives and chronic disease management programs.
Duties & Responsibilities
- Provide high-quality, patient-centered primary care for individuals and families.
- Conduct comprehensive health assessments, diagnosis, and treatment planning.
- Coordinate care with on-site behavioral health professionals and social workers.
- Participate in care management and preventive services, including diabetes education and management.
- Maintain patient caseload of 16-22 patients per day.
- Document encounters and care plans accurately and in a timely manner.
- Option to work a 4-day or 5-day week; office hours are typically Monday through Friday, 8 a.m. to 5 p.m.
- Minimal call coverage required (2-3 weeks per year), with no hospital rotation.
- Engage in ongoing professional development and continuous quality improvement initiatives.
Salary & Benefits
- Competitive base salary starting at $250,000, with additional RVU-based incentives.
- Potential total compensation up to $400,000 based on production.

Qualifications & Requirements
