Key Responsibilities
- Deliver comprehensive primary care to a diverse Medicare patient panel, focusing on prevention, chronic disease management, and evidence-based interventions.
- Work collaboratively within a multidisciplinary care team, including scribes, medical assistants, social workers, and nurse practitioners, to ensure coordinated, patient-centered care.
- Utilize on-site laboratory services and electronic medical records (EMR) systems such as Canopy and Greenway to document, track, and optimize patient outcomes.
- Provide patient visits averaging 20 minutes for routine appointments and 40 minutes for new or complex cases, ensuring thorough evaluation and personalized care plans.
- Engage in value-based care initiatives aimed at improving quality metrics, reducing unnecessary hospital admissions, and enhancing patient satisfaction.
- Participate in community-focused activities and wellness programs within the clinic, including exercise sessions, events, and educational opportunities for patients.
- Access specialist consultation platforms and collaborate as needed to support comprehensive patient management.
- Contribute to a collegial environment with 3-5 providers per clinic, fostering teamwork and knowledge sharing.
Qualifications
- Graduate of an accredited medical school with a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Spanish language proficiency is required to meet the needs of the local patient community.
- Fellowship training in Geriatrics is preferred but not mandatory.
- Applicants must not be current residents; this position is intended for licensed, practicing physicians.
