The Primary Care Physician (PCP) role in Chicago, IL, offers an excellent opportunity for dedicated medical professionals looking to make a significant impact through value-based healthcare. As a full-time member of an innovative network of neighborhood primary care centers, the Primary Care Physician will provide high-quality, equitable care to local Medicare patient populations. Supported by a collaborative care team, the PCP can focus on patient wellness and preventive care, contributing to improved health outcomes and patient satisfaction in the Chicago area.
Key Responsibilities
- Deliver comprehensive primary care to a diverse Medicare patient population, focusing on preventive medicine, chronic disease management, and long-term wellness.
- Work collaboratively within a multidisciplinary care team, including scribes, medical assistants, and social workers, to ensure coordinated and effective patient care.
- Utilize on-site laboratory facilities and electronic medical record (EMR) systems, including Canopy and Greenway, to support diagnosis and treatment plans.
- Participate in a value-based healthcare model designed to improve patient outcomes, reduce hospital admissions, and enhance the overall patient experience.
- Manage an average daily patient volume of 13–16, with regular visits scheduled for 20 minutes and comprehensive diagnostic or new patient visits scheduled for 40 minutes.
- Engage with patients in community activities and educational events offered at the clinic, supporting holistic wellness initiatives.
- Coordinate with three to five providers per clinic, including other Primary Care Physicians and Nurse Practitioners.
- Ensure timely and effective communication with patients, families, and care team members regarding care plans and follow-up.
Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Spanish language proficiency required to better serve the patient community.
- Fellowship training in Geriatrics is preferred but not required.
- No residents will be considered for this position.
