Our modern clinics provide on-site laboratory services with skilled phlebotomists, and access to RubiconMD, empowering you with specialist consultation support. With a typical patient volume of 13-16 per day, you will have ample time for both regular and comprehensive diagnostic visits, enhancing the quality of your interactions and the care you provide. The average team includes 3-5 providers per clinic, fostering a collaborative and collegial work atmosphere.

Primary Care Physician (PCP) – Summary
- Deliver patient-centered, value-based primary care to Medicare populations.
- Work collaboratively within an interdisciplinary care team to enhance patient outcomes.
- Utilize on-site laboratory and specialist consult resources to optimize patient management.
- Focus on preventive medicine and reducing hospital admissions in a supportive clinical environment.
Duties & Responsibilities
- Conduct comprehensive patient assessments, develop individualized care plans, and manage chronic and acute conditions.
- Perform regular and full diagnostic visits (20–40 minutes per visit), ensuring thorough and attentive care.
- Collaborate with a multidisciplinary team including scribes, medical assistants, and social workers.
- Implement value-based care principles to drive quality outcomes and patient engagement.
- Document all patient encounters in EMR systems (Canopy and Greenway).
- Participate in community health initiatives, including patient activities and wellness events.
- Support patient transportation needs and facilitate access to care.
- Engage in professional development through CME opportunities and educational activities.

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) degree required.
- Board certification (or board eligibility) in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license (non-probationary).
- Bilingual proficiency in Spanish strongly preferred to serve diverse communities.
- Fellowship training in Geriatrics preferred but not required.
- No current residents; open to experienced physicians.
