We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our innovative network of neighborhood primary care centers in Chicago, IL, specifically serving the Chinatown community. As a full-time Primary Care Physician (PCP) in our organization, you will play a vital role in providing equitable, patient-centered, value-based healthcare to local Medicare patient populations. This position is ideal for physicians who are committed to preventive medicine and are passionate about delivering an unmatched patient experience.
Responsibilities
- Provide comprehensive primary care for a panel of Medicare patients, focusing on preventive health, management of chronic conditions, and acute care needs.
- Work within a value-based healthcare model designed to achieve high quality outcomes and improved patient satisfaction.
- Collaborate closely with a large, multidisciplinary care team, including scribes, medical assistants, and social workers, to ensure holistic patient care.
- Manage a daily patient volume of approximately 13-16 patients, with 20-minute appointments for regular visits and 40-minute appointments for full diagnostics and new patients.
- Utilize on-site laboratory services and access to specialist consultations via RubiconMD to enhance patient care delivery.
- Document patient encounters and manage clinical workflows using Canopy and Greenway electronic medical record (EMR) systems.
- Participate in community engagement through activities and events held in the clinic's community room, supporting the overall health and wellness of patients.
- Contribute to a collaborative clinical environment, typically working alongside 3-5 providers per clinic, including Nurse Practitioners (NPs) and other Primary Care Physicians (PCPs).
- Emphasize preventive care, reduction in hospital admissions, and improvement in quality metrics through a value-based health model.
Qualifications
- Graduate of an accredited Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) program.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary medical license in the state of Illinois.
- Proficiency in Cantonese is required to serve the needs of our diverse patient population in Chicago’s Chinatown area.
- Fellowship training in Geriatrics is preferred, but not required.
- This opportunity is not open to residents; only fully trained physicians will be considered.
