We are seeking a dedicated and compassionate Primary Care Physician (PCP) to join our innovative network of neighborhood primary care centers in Gary, IN. As a full-time Primary Care Physician, you will deliver equitable, patient-centered, and value-based healthcare to the local Medicare patient population. Primary Care Physician (PCP) - Summary
- Deliver comprehensive, value-based primary care to a diverse Medicare patient population.
- Work collaboratively within a multidisciplinary care team to optimize patient outcomes.
- Focus on preventive medicine, patient education, and quality improvement.
- Practice in a modern, well-equipped clinic with access to on-site labs and specialist consultations.
- Participate in an innovative healthcare model designed to reward quality of care and patient satisfaction.
Duties & Responsibilities
- Provide direct patient care, including diagnosis, treatment, and management of acute and chronic conditions for Medicare patients.
- Conduct thorough patient visits: approximately 13-16 patients per day, with 20-minute regular visits and 40-minute comprehensive assessments for new or complex cases.
- Collaborate with a care team that includes scribes, medical assistants, and social workers to ensure coordinated and holistic care.
- Utilize electronic medical records (EMR) systems such as Canopy and Greenway for accurate and efficient documentation.
- Engage in value-based care initiatives, focusing on reducing hospital admissions and improving preventive care metrics.
- Participate in community events and activities held in the clinic’s community room, promoting patient wellness and engagement.
- Leverage on-site resources such as laboratory services and specialist access through RubiconMD.
- Support transportation needs of patients within five miles of the clinic, ensuring accessibility to care.
- Contribute to a positive, team-oriented clinic culture and participate in ongoing quality improvement initiatives.
Salary & Benefits
- Competitive base salary up to $245,000, with additional incentive bonus based on quality metrics and outcomes.

Qualifications & Requirements
- Medical Doctor (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree from an accredited institution.
- Board certified or board eligible in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Fellowship training in Geriatrics preferred, but not required.
- No residents; seeking fully credentialed physicians.
