As a Primary Care Physician (MD/DO) – Value-Based Care, you will join an innovative healthcare organization dedicated to providing high-quality, patient-centered preventive primary care to adults aged 65 and older. Our care model is designed to ensure physicians have ample time with each patient, with daily patient census and visit lengths that support comprehensive, preventive, and chronic care management. As part of an integrated care team, you will collaborate with nurse practitioners, physician assistants, medical assistants, social workers, and specialists in lab and pharmacy services to deliver holistic care to a predominantly Medicare Advantage patient population. We seek board certified or board eligible Internal Medicine or Family Medicine physicians who are mission-driven, team-oriented, and passionate about improving the quality of life for seniors. While geriatric and HEDIS experience are appreciated, they are not required, as full training is provided. Compensation is highly competitive, with a comprehensive benefits package, including occurrence-based malpractice coverage, performance incentives, and relocation assistance for qualified candidates. Primary Care Physician (MD/DO) – Value-Based Care Summary- Deliver comprehensive, preventive primary care to adults aged 65+, primarily Medicare Advantage patients
- Full support from an integrated care team including NP/PA, medical assistants, social workers, and pharmacy staff
- Practice medicine with a focus on quality, outcomes, and patient relationships, free from RVU pressure
- Opportunity to develop expertise in value-based care with structured onboarding and ongoing professional development
- Provide direct patient care to a panel of adults 65+, focusing on prevention and chronic disease management
- Conduct thorough patient assessments during 20-40 minute visits, ensuring personalized care plans
- Collaborate with an interdisciplinary team to coordinate care and address all aspects of patient health
- Utilize electronic medical records (EMR) efficiently; participate in EMR optimization initiatives
- Engage in ongoing value-based care training and implement best practices in documentation and clinical workflows
- Proactively participate in quality improvement activities and population health initiatives

- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Internal Medicine or Family Medicine
