In this full-time Primary Care Physician (MD/DO) position, you will care for a senior patient population with complex medical needs. You will work collaboratively with a dedicated care team to ensure each patient receives individualized, high-quality, and coordinated care. Primary Care Physician (MD/DO) – Summary
- Deliver outpatient primary care to a senior patient population.
- Practice medicine within a value-based care model focused on patient satisfaction, clinical compliance, and reducing hospitalizations.

Duties & Responsibilities
- Conduct comprehensive assessments, diagnosis, and management of acute and chronic conditions in an outpatient setting.
- Develop, implement, and monitor individualized care plans for each patient.
- Manage a patient panel size with a maximum of 650 patients, typically seeing 10-12 patients per day.
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, referral specialists, and leadership support.
- Utilize office procedures such as suturing, joint injections, skin biopsies, and basic wound care, as needed.
- Participate in call rotation as required by clinic staffing levels, with a favorable call schedule based on group size.
- Engage in ongoing quality improvement initiatives, including HEDIS compliance and patient satisfaction enhancements.
- Promote patient education, preventive health, and community wellness programs within the clinic.
- Maintain accurate and timely documentation in the electronic medical record system.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not mandatory.
- Current, active, and unrestricted medical license to practice.
- Valid DEA registration.
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Commitment to providing compassionate, patient-centered care to a diverse senior population.
- Excellent communication and teamwork skills.
- Strong computer literacy and ability to utilize electronic health records efficiently.
