Primary Care Physician (MD/DO) – PCP
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team. As a Primary Care Physician, you will play a critical role in delivering comprehensive outpatient primary care to a diverse senior patient population. The Primary Care Physician (MD/DO) will be responsible for managing a patient panel primarily consisting of seniors living with multiple complex chronic conditions, such as heart disease, diabetes, and hypertension. The value-based care model implemented at our clinics emphasizes quality over quantity, allowing you to spend more time with each patient and focus on preventive care, holistic management, and improved patient satisfaction. You will work collaboratively with a dedicated care team, which includes a medical assistant, scheduler, referral specialist, and full leadership support, ensuring you have the resources to deliver exceptional care.
As a Primary Care Physician (MD/DO), you will benefit from a manageable patient load, typically seeing 10-12 patients per day with a maximum panel size of 650. The position offers a predictable Monday through Friday schedule, allowing for work-life balance. Call responsibilities are shared across the provider team and are based on the number of physicians in the market, contributing to a sustainable workload.

Primary Care Physician (MD/DO) – PCP Summary- Deliver outpatient, value-based primary care to a senior patient population with complex health needs
- Collaborate within a team-based care model supported by onsite and leadership resources

- Provide comprehensive primary care services to adult and senior patients in an outpatient setting
- Manage a panel of up to 650 patients, focusing on those with multiple chronic conditions
- Conduct thorough assessments, routine exams, chronic disease management, and preventive care
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists
- Utilize value-based care principles to improve patient outcomes and satisfaction
- Contribute to reducing hospital admissions and improving health metrics such as HEDIS compliance
- Participate in call rotation as assigned within the provider team
- Engage in ongoing professional development and adhere to best clinical practices
- Document patient encounters and treatment plans accurately in the electronic health record

- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted license to practice medicine
- Current DEA registration
- Proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is a plus but not required
- Demonstrates a passion for senior care and value-based medicine
- Exceptional interpersonal communication and team collaboration skills
- Comfortable working with a diverse patient population with complex health needs
- Motivated to contribute positively to patient satisfaction and clinical outcomes
- Adaptable and open to innovative healthcare delivery models
