Primary Care Physician (MD/DO) – PCP Summary- Outpatient primary care for senior patients with complex health conditions

- Provide comprehensive primary care to a panel of up to 650 senior patients
- Manage an average of 10–12 patients per day, ensuring individualized attention
- Lead a care team including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives and meet performance metrics such as HEDIS compliance
- Deliver care in accordance with organizational values and patient satisfaction goals
- Utilize clinical skills in office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Engage in limited call schedule, determined by market provider numbers
- Support patient engagement through wellness activities and accessible services

- MD or DO degree with active, unrestricted medical license
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus but not required
- Current DEA registration
- Proficient in office-based procedures: suturing, joint injections, skin biopsies, basic wound care
