As a Primary Care Physician (MD/DO), you will care for a panel of adult and senior patients, many with multiple chronic conditions such as heart disease, diabetes, and hypertension. You will work weekday hours, enjoy a manageable patient load, and have access to a dedicated care team including medical assistants, schedulers, referral specialists, and ongoing leadership support. Our new center, opening May 2025, is equipped with state-of-the-art facilities and provides added conveniences for patients like transportation services and on-site wellness activities such as exercise and art classes to promote holistic health at no additional cost.

Primary Care Physician (MD/DO) – Outpatient, Value-Based Care Summary
- Provide comprehensive outpatient primary care to adult and senior patients, focusing on those with three to five chronic conditions.
- Work within a value-based care model to improve patient outcomes and satisfaction.
- Collaborate with a dedicated support team and utilize advanced clinic resources.
- Participate in a culture of innovation, respect, and community wellness.
- Clinic operates Monday through Friday, 8 am to 5 pm.
Duties & Responsibilities
- Deliver high-quality, patient-centered primary care to an outpatient panel of up to 650 patients.
- Diagnose and manage chronic diseases (e.g., heart disease, diabetes, hypertension) in a senior population.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Work with a multidisciplinary care team including medical assistants, schedulers, and referral specialists.
- Maintain compliance with HEDIS measures and other clinical quality benchmarks.
- Participate in call rotations as determined by the number of local providers.
- Document patient encounters thoroughly and accurately within electronic health records.
- Engage patients in preventive care, wellness activities, and community health programs.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship in Geriatrics is a plus but not required.
- Current, unrestricted, and active medical license.
- Current DEA registration.
- Proficient in office-based procedures relevant to primary care.
