This Primary Care Physician (MD/DO) position plays a crucial role in advancing our mission to deliver best-in-class healthcare services. Physicians are supported by dedicated care teams, including a medical assistant, scheduler, referral specialist, and robust leadership guidance, creating an empowering and collaborative work environment.
As a Primary Care Physician (MD/DO) – Outpatient FM/IM, you will care for a panel of seniors with complex health needs, emphasizing preventive care, chronic disease management, and patient education. Primary Care Physician (MD/DO) – Outpatient FM/IM Summary- Outpatient primary care position for Family Medicine (FM) or Internal Medicine (IM) physicians
- Focus on value-based care, serving a panel of senior patients with multiple chronic conditions
- Collaborative team environment with comprehensive support resources
- Innovative care delivery model emphasizing patient engagement and wellness
- Located in a vibrant community with diverse patient populations
- Deliver comprehensive primary care to a panel of senior patients, averaging 10-12 patients per day
- Provide preventive care, chronic disease management, and patient education
- Utilize a value-based care approach to improve patient outcomes and satisfaction
- Participate in activities to promote wellness and community engagement, including on-site events
- Collaborate with a dedicated care team (medical assistant, scheduler, referral specialist, and leadership)
- Maintain accurate and timely clinical documentation
- Participate in regular call schedules as determined by the provider group
- Adhere to clinical, ethical, and regulatory standards of practice
- Base salary of $260,000 with total earning potential over $300,000, including performance bonuses
- Bonuses based on patient satisfaction, company values, clinical compliance, and reduced hospitalizations

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus, but not required
- Current, unrestricted, active medical license
- Current DEA registration
- Proficiency in common office procedures: suturing, joint injections, skin biopsies, basic wound care
- Demonstrates passion for primary care and value-based healthcare delivery
- Experienced or interested in caring for seniors with complex medical needs
- Strong interpersonal and communication skills for building long-term patient relationships
- Comfortable working in a collaborative, multidisciplinary team environment
- Committed to continuous improvement and delivering high-quality, patient-centered care
