Primary Care Physician (MD/DO) – Summary- Deliver comprehensive, preventive, and chronic care to adults 65+ in a value-based care environment.
- Work full-time with no on-call responsibilities, allowing for optimal work-life balance.
- Serve a patient panel primarily consisting of Medicare Advantage members with a focus on preventive health and disease management.
- Benefit from a collaborative, multidisciplinary team dedicated to exceptional patient outcomes.
- Provide primary care services to adults aged 65 and older, focusing on preventive medicine and chronic disease management.
- Establish and maintain meaningful patient relationships, dedicating 20-40 minutes per visit for thorough, personalized care.
- Work collaboratively within an integrated care team including NPs/PAs, medical assistants, social workers, lab, pharmacy, and referral coordinators.
- Participate in a comprehensive onboarding and ongoing professional development program emphasizing value-based care principles.
