Primary Care Physician (MD/DO, Internal Medicine or Family Medicine) – Summary
- Provide comprehensive patient care as a Primary Care Physician (MD/DO, Internal Medicine or Family Medicine) within a value-based, team-oriented environment
- Deliver coordinated, patient-centered healthcare through a multidisciplinary approach
- Contribute to a mission-driven organization dedicated to superior patient satisfaction and well-being
Duties & Responsibilities
- Provide primary care services, including preventive, acute, and chronic disease management to adult patients
- Conduct patient consultations, develop care plans, and follow up on health outcomes
- Collaborate closely with APRNs, RNs, and Quality Improvement teams to coordinate patient care and ensure best practices
- Utilize robust transitional care and high-risk outreach programs to support patients with complex health needs
- Participate in quality improvement initiatives and receive consistent feedback on clinical performance and patient outcomes
- Maintain accurate and timely patient records in compliance with organizational policies and regulatory standards
- Foster a supportive, inclusive, and patient-focused environment

Qualifications & Requirements
- Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.) degree
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted medical license
- Valid DEA and CDS licenses, or ability to obtain
- Preferred: 1+ years of clinical experience in adult, family, or geriatric care
- Preferred: Basic Cardiac Life Support (BCLS) certification
