Primary Care Physician (MD/DO, Internal Medicine or Family Medicine) – Summary- Full-time primary care opportunity in a value-based care delivery organization
- Physician-led, patient-centered team environment

- Autonomy in clinical practice, supported by a multidisciplinary care team

- Provide comprehensive primary care services to adult patients, focusing on wellness, prevention, and chronic disease management
- Conduct patient evaluations, develop treatment plans, and coordinate care with interdisciplinary team members
- Utilize evidence-based medicine to deliver high-quality, cost-effective care
- Participate in quality improvement initiatives and receive ongoing performance feedback
- Engage in robust transitional care and outreach programs for high-risk populations
- Maintain accurate and up-to-date patient records in the electronic health system
- Collaborate with APRNs, RNs, and the Quality Improvement team to optimize patient outcomes
- Uphold the highest standards for ethical and professional conduct

- M.D. or D.O. degree required
- Board Certified or Board Eligible in Family Medicine or Internal Medicine
- Active, unrestricted Louisiana medical license
- Valid DEA and CDS licenses, or ability to obtain
- Preferred: Minimum of 1 year clinical experience in adult, family, or geriatric care
- Preferred: Basic Cardiac Life Support (BCLS) certification
