As a Primary Care Physician, you will be at the center of a multidisciplinary team, delivering comprehensive outpatient care for a senior patient population. Primary Care Physician - Summary
- Provide high-quality primary care services in an outpatient setting, focusing on a senior patient population with multiple complex conditions.
- Work collaboratively with a dedicated care team, supported by medical assistants, schedulers, referral specialists, and leadership.
- Practice within a value-based care model that emphasizes preventive medicine, continuity of care, and patient engagement.
Duties & Responsibilities
- Perform comprehensive medical assessments, diagnosis, and treatment of patients with chronic and acute illnesses.
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day, to ensure personalized and attentive care.
- Develop and implement individualized care plans for seniors with complex medical needs, including heart disease, diabetes, hypertension, and more.
- Coordinate care with other healthcare professionals and provide referrals as needed for specialized services.
- Utilize electronic health records for accurate documentation and continuity of care.
- Execute office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Participate in quality improvement initiatives, focusing on HEDIS compliance, patient satisfaction, and reduction of hospitalizations.
- Engage with patients and families to educate them about wellness, disease prevention, and healthy lifestyle choices.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not required.
- Active, unrestricted medical license and DEA registration.
- Proficient in office procedures such as suturing, joint injections, skin biopsies, and wound care.
- Strong interpersonal and communication skills to foster patient trust and collaboration within the care team.
- Commitment to delivering compassionate, patient-centric care in an outpatient, value-based setting.
