Primary Care Physician (MD/DO) – Internal Medicine / Family Medicine - Summary
- Full-time primary care position in an outpatient, value-based setting
- Serve an adult patient population with complex chronic conditions
- Work within a supportive, multidisciplinary care team
- Focus on long-term, relationship-based patient care
Duties & Responsibilities
- Provide comprehensive primary care to a panel of senior patients with multiple chronic conditions, including heart disease, diabetes, and hypertension
- Deliver care in an outpatient setting with regular Monday–Friday hours
- Utilize a value-based care model to maximize patient outcomes and satisfaction
- Collaborate with medical assistants, schedulers, referral specialists, and leadership to coordinate patient care
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Participate in a call schedule proportionate to the number of providers in the market
- Ensure compliance with HEDIS measures and contribute to reducing hospitalizations among your patient panel
- Engage patients in wellness activities and coordinate ancillary services as needed

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board Certification or Board Eligibility in Internal Medicine (IM) or Family Medicine (FM) required
- Active, unrestricted medical license and DEA registration
- Fellowship training in Geriatrics is a plus, but not required
- Demonstrated proficiency in common office-based clinical procedures
- Commitment to value-based, patient-centered care
