We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Philadelphia, PA. As a Primary Care Physician, you will work collaboratively with a multidisciplinary care team that includes medical assistants, schedulers, referral specialists, and supportive leadership. Primary Care Physician (MD/DO) – Internal Medicine / Family Medicine Summary

Duties & Responsibilities
- Provide comprehensive primary care services to adult and senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension
- Maintain a patient panel with an average of 10-12 patients per day, up to a maximum panel size of 650
- Utilize a value-based approach to care, emphasizing preventive medicine, health outcomes, and patient satisfaction
- Lead a dedicated care team that includes a medical assistant, scheduler, and referral specialist
- Participate in care team meetings, contribute to quality improvement initiatives, and adhere to company values and protocols
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Document all encounters and follow-up actions in the electronic health record system
- Participate in a shared call schedule, based on the number of providers in the market (currently 1:32 weeks)
- Promote patient wellness by encouraging participation in onsite activities such as exercise and arts programs
Salary & Benefits
- Competitive base salary of $260,000 with first-year earning potential exceeding $300,000
- Performance-based bonuses up to 20% of base salary, tied to patient satisfaction, alignment with organizational values, HEDIS compliance, and reduced hospitalizations

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship training in Geriatrics is a plus, but not required
- Current, unrestricted, and active license to practice medicine
- Active DEA registration
- Proficiency in basic office-based procedures, including suturing, joint injections, skin biopsies, and wound care
