Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) - Summary
- Deliver compassionate, evidence-based care to a senior patient population with complex medical conditions
- Practice in a value-based care model focused on patient outcomes and satisfaction
- Work collaboratively within a multidisciplinary care team that includes medical assistants, schedulers, referral specialists, and leadership support
- Contribute to a culture of wellness and continuous improvement in primary care delivery
Duties & Responsibilities
- Provide comprehensive primary care services in an outpatient setting, focusing on seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension
- Manage a panel size of up to 650 patients, averaging 10-12 patient visits per day for individualized attention and continuity of care
- Participate actively in value-based care initiatives, ensuring adherence to quality standards such as HEDIS compliance
- Foster strong patient relationships, promoting satisfaction and engagement with health management plans
- Utilize office-based procedural skills, including suturing, joint injections, skin biopsies, and basic wound care
- Engage with the clinic's wellness programs and support patient participation in activities designed to enhance physical and mental health
- Collaborate with care team members to coordinate referrals, follow-ups, and comprehensive patient support
- Participate in a call rotation schedule, proportionate to the number of physicians in the market

Salary & Benefits
- Base compensation starting at $260,000, with total earning potential exceeding $312,000 annually
- Performance-based bonuses up to 20% of base salary, tied to patient satisfaction, adherence to company values, HEDIS compliance, and reduced hospitalizations

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) required
- Active, unrestricted medical license and current DEA registration
- Fellowship training in Geriatrics is a plus, but not required
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Demonstrated commitment to patient-centered, value-based care
- Strong interpersonal and communication skills
