In this Primary Care Physician role, you will be responsible for managing a panel of senior patients, focusing on quality care rather than volume. Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) - Summary
- Provide outpatient primary care to a panel of senior patients with complex health needs.
- Work within a value-based care model focused on quality, patient satisfaction, and health outcomes.
- Collaborate with a dedicated care team and supportive leadership.

Duties & Responsibilities
- Deliver comprehensive primary care in an outpatient setting, managing a panel of up to 650 patients.
- Average 10-12 patient visits per day, allowing for quality-focused interactions.
- Utilize a value-based approach to care, including monitoring HEDIS compliance and improving patient satisfaction.
- Collaborate with care team members including medical assistants, schedulers, and referral specialists.
- Practice office-based procedures such as suturing, joint injections, skin biopsies, and wound care as needed.
- Participate in call schedule based on the number of providers in the market.
- Encourage patient participation in wellness programs, including exercise and art classes.
- Support the organizational mission of improving the health and wellness of the senior community.

Qualifications & Requirements
- MD or DO degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine.
- Current DEA registration.
- Proficient in office-based procedures: suturing, joint injections, skin biopsies, basic wound care.
- Fellowship training in Geriatrics is a plus but not required.
