Primary Care Physician (MD/DO, FM/IM, Outpatient, Value-Based Care) - Summary

Duties & Responsibilities
- Deliver comprehensive primary care to a senior patient population, typically managing 3-5 complex chronic conditions per patient (e.g., heart disease, diabetes, hypertension)
- Maintain a manageable daily patient census (average 10-12 per day) to enable quality care and relationship building
- Utilize a value-based care approach, focusing on preventive health, chronic disease management, and reducing unnecessary hospitalizations
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists for seamless patient support
- Participate in continuous quality improvement initiatives and adhere to HEDIS compliance standards
- Engage in patient education, care coordination, and community wellness initiatives
- Participate in a shared call schedule, with frequency based on the number of providers in the market
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed
- Document all patient interactions and care plans accurately in the electronic medical record

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Unrestricted, active medical license and current DEA registration
- Proficient in performing office procedures (suturing, joint injections, skin biopsies, basic wound care)
- Fellowship in Geriatrics is a plus but not required
- Strong commitment to patient-centered, value-based healthcare
