As a Primary Care Physician (MD/DO, FM/IM) in Port Charlotte, FL, you will provide outpatient care for a panel of senior patients, typically managing 10-12 visits per day and a maximum panel size of 650. You will be supported by a dedicated care team, including a medical assistant, scheduler, referral specialist, and engaged leadership. The position is Monday through Friday, 8 AM to 5 PM, with a call schedule distributed equitably among providers in the area. Duties & Responsibilities
- Deliver comprehensive primary care to seniors with multiple chronic conditions (e.g., heart disease, diabetes, hypertension)
- Manage a patient panel of up to 650, averaging 10-12 patient visits per day
- Work collaboratively with a dedicated care team for optimal patient support
- Utilize office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Participate in a rotating call schedule shared across the provider group
- Emphasize patient satisfaction, adherence to company values, and compliance with HEDIS measures
- Contribute to reducing hospital admissions and sick days through proactive, preventive care
- Engage with patients in wellness programs and activities designed to promote better health outcomes

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship training in Geriatrics is a plus but not required
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in basic in-office procedures (suturing, joint injections, skin biopsies, wound care)
