Primary Care Physician (MD/DO, FM/IM) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity at our innovative healthcare clinic in Port Charlotte, FL. Responsibilities
- Deliver comprehensive primary care to a panel of up to 650 patients, primarily seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Provide care in an outpatient setting, with an average daily patient volume of 10-12, ensuring personalized attention for each individual.
- Collaborate with a dedicated care team including a medical assistant, scheduler, referral specialist, and supportive leadership staff to optimize patient outcomes.
- Adhere to a value-based care model, focusing on patient satisfaction, clinical quality (including HEDIS compliance), and reducing hospitalizations.
- Participate in a rotating call schedule, determined by the total number of physicians in the market (example: call is 1 in 32 weeks if 32 providers are present).
- Perform common office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Engage in activities and wellness programs designed to enhance patients’ overall health, including coordination with community resources and in-clinic programs.
- Maintain accurate and timely documentation of patient encounters and care plans in accordance with regulatory and organizational requirements.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficiency in office-based clinical procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus but not required.
- Strong commitment to patient-centered, value-based care and a collaborative team environment.
