Primary Care Physician (MD/DO, FM/IM) OverviewWe are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) for a full-time, outpatient opportunity in St. Petersburg, FL. This role is an excellent fit for physicians who are passionate about value-based care and delivering holistic, patient-centered medical services to a diverse senior population. As part of our innovative healthcare team, you will focus on improving outcomes for patients with complex medical needs while working within a supportive, collaborative environment.
Responsibilities- Provide comprehensive outpatient primary care to an assigned panel of senior patients, primarily those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care under a value-based model, focusing on patient satisfaction, adherence to clinical best practices, and reduction of hospital admissions.
- Manage a panel size of up to 650 patients, averaging 10-12 patient visits per day to ensure thorough, high-quality encounters.
- Utilize a dedicated care team including a medical assistant, scheduler, referral specialist, and full leadership support to optimize care delivery and coordination.
- Participate in a collaborative call schedule, with call frequency based on the number of providers in the local market.
- Promote patient engagement in available wellness programs, such as exercise and art activities, tailored to support the health of senior patients at no additional cost to them.
- Start date for this Primary Care Physician (MD/DO, FM/IM) role is anticipated for May 2025 with the opening of our new center in St. Petersburg, FL.
- Ensure compliance with HEDIS measures and other quality benchmarks as part of our commitment to excellence in patient care.
- Perform minor office procedures including suturing, joint injections, skin biopsies, and basic wound care as clinically indicated.
- Medical degree (MD or DO) from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) is required for the Primary Care Physician (MD/DO, FM/IM) position.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficient in common office-based procedures and electronic medical record systems.
