Primary Care Physician (MD/DO, FM/IM) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Philadelphia, PA. As a Full-time practitioner, you will provide high-quality, outpatient primary care to a senior patient population within a value-based care model. Our organization is committed to delivering comprehensive and patient-centered healthcare, fostering a supportive environment where both patients and staff thrive.
Key Responsibilities
- Deliver exceptional, patient-focused primary care services in an outpatient setting, primarily serving seniors with multiple complex conditions such as heart disease, diabetes, and hypertension.
- Manage an average patient load of 10-12 individuals per day, ensuring continuity and quality of care for a panel of up to 650 patients.
- Collaborate with a dedicated care team including medical assistants, schedulers, referral specialists, and supportive leadership to optimize patient outcomes.
- Participate in a value-based care model, contributing to patient satisfaction, adherence to HEDIS compliance standards, and reduction in hospital sick days.
- Maintain regular clinic hours, Monday through Friday from 8:00 AM to 5:00 PM, with a shared call schedule based on the number of providers in the market.
- Utilize proficient clinical skills in office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Support the company’s mission of building a wellness-focused community by promoting and participating in activities designed for senior patients, such as exercise and art programs.
Qualifications
- Medical Degree (MD or DO) with current, unrestricted, and active license to practice medicine in the United States.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus, but not required.
- Current DEA registration.
- Demonstrated proficiency in office-based procedures, including wound care and minor surgical skills.
