As a valued Primary Care Physician (MD/DO, FM/IM), you will provide outpatient care in a modern, value-based care model, serving a senior patient population with complex healthcare needs.
Responsibilities- Deliver comprehensive outpatient primary care to a panel of senior patients, typically managing 10-12 patients per day.
- Work within a value-based care model, emphasizing preventive care, HEDIS compliance, and reducing hospitalizations.
- Provide continuity of care to a maximum panel size of 650 patients, focusing on chronic disease management, including heart disease, diabetes, and hypertension.
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and strong leadership support.
- Engage in patient-centered activities and promote health initiatives, such as exercise and art programs tailored to seniors.
- Participate in a rotating call schedule based on the number of providers in the market (e.g., 1:32 weeks if 32 physicians are present).
- Maintain high standards of patient satisfaction and exemplify the organization’s core values.
- Perform office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship in Geriatrics is a plus but not required.
- Active, unrestricted medical license and current DEA registration.
- Proficiency in office-based procedures relevant to primary care.
