Primary Care Physician (MD/DO, FM/IM) - Summary
- Outpatient primary care for a senior patient population
- Full-time role in Philadelphia, PA

Duties & Responsibilities
- Provide comprehensive primary care to senior patients, many with multiple chronic conditions (e.g., heart disease, diabetes, hypertension)
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day
- Deliver care in accordance with value-based care principles, focusing on patient satisfaction and improved health outcomes
- Collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Participate in a call schedule shared among providers (frequency based on provider count in the market)
- Utilize skills in office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Contribute to organizational goals by adhering to HEDIS compliance and reducing hospital sick days
- Embody and promote the organization’s values in all patient and staff interactions

Qualifications & Requirements
- MD or DO degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship training in Geriatrics is a plus, but not required
- Active, unrestricted medical license in Pennsylvania
- Current DEA registration
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
