As a Primary Care Physician, you will work closely with a dedicated medical assistant, scheduler, and referral specialist, as well as receive full backing from our experienced leadership. This collaborative model enables providers to focus on what matters most—delivering outstanding care and building meaningful relationships with patients, primarily seniors managing multiple chronic conditions such as heart disease, diabetes, and hypertension.

Primary Care Physician (MD/DO, FM/IM) - Summary
- Full-time, outpatient primary care role in a value-based care setting

Duties & Responsibilities
- Provide comprehensive outpatient primary care to a panel of up to 650 patients, averaging 10-12 patients per day
- Emphasize value-based care with a focus on patient satisfaction, health outcomes, and reduced hospitalizations
- Collaborate with an assigned care team, including a medical assistant, scheduler, and referral specialist
- Participate in call schedule as determined by the number of providers in the market (example: 1:32 weeks)
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and wound care
- Contribute to the clinic’s mission by promoting wellness programming and community engagement
- Maintain compliance with HEDIS and organizational quality standards
- Document and manage patient care using electronic medical records
Salary & Benefits
- Base salary of $270,000, with total potential compensation up to $324,000 (including bonuses)
- Bonuses based on patient satisfaction, adherence to company values, HEDIS compliance, and reducing hospital sick days

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Board certification or board eligibility in Family Medicine (FM) or Internal Medicine (IM) required
- Active, unrestricted medical license and current DEA registration
- Proficiency in performing outpatient procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Fellowship in Geriatrics is a plus, but not required
- Strong commitment to patient-centered, value-based care
