Primary Care Physician (MD/DO, FM/IM) Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join an innovative healthcare team in Phoenix, AZ. This full-time position is based in a modern, patient-centered outpatient clinic that specializes in providing exceptional value-based care for the senior population. Key Responsibilities
- Provide primary care services in an outpatient setting, with a focus on adult and senior patients presenting with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Maintain a manageable caseload of 10-12 patients per day, ensuring personalized and attentive care for each patient.
- Manage a patient panel size of up to 650 individuals, focusing on continuity of care and proactive health management.
- Work collaboratively within a value-based care model, emphasizing patient outcomes, satisfaction, and quality of life.
- Utilize a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support, to optimize patient experiences and clinical outcomes.
- Participate in a rotating call schedule based on the number of providers in the market (e.g., a 1:32 week call schedule if 32 physicians are present).
- Promote wellness and community engagement by supporting clinic-based activities and resources designed for seniors, such as exercise and art classes.
- Adhere to company values and contribute to high patient satisfaction ratings and compliance with quality metrics, including HEDIS measures and reduction of hospital admissions.
- Perform office-based procedures as appropriate, such as suturing, joint injections, skin biopsies, and basic wound care.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Fellowship training in Geriatrics is advantageous but not required.
- Proficiency in basic office procedures, including suturing, joint injections, skin biopsies, and wound care.
