Primary Care Physician (MD/DO, FM/IM) – Summary
- Provide comprehensive outpatient primary care to a senior patient population with chronic and complex health needs.
- Deliver patient-centered, value-based care focused on quality and outcomes, not just patient volume.
- Engage in a collaborative, multidisciplinary team environment with strong administrative and clinical support.
- Participate in an innovative care model that integrates wellness activities and patient transportation support.
- Full-time position based in Phoenix/Mesa E., AZ.
Duties & Responsibilities
- Conduct outpatient primary care visits for 10-12 senior patients per day, focusing on continuity and quality of care.
- Manage a patient panel of up to 650, predominantly adults with multiple chronic diseases such as heart disease, diabetes, and hypertension.
- Work collaboratively with a dedicated care team that includes a medical assistant, scheduler, referral specialist, and leadership support.
- Utilize value-based care principles, aiming to improve patient satisfaction, compliance, and health outcomes while reducing unnecessary hospitalizations.
- Participate in on-call rotation in coordination with other providers in the market.
- Maintain accurate patient records and uphold all clinical protocols and ethical standards.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Contribute to a culture of respect, collaboration, and continuous quality improvement.

Qualifications & Requirements
- Medical degree (MD or DO) required.
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship or experience in Geriatrics is a plus but not mandatory.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
