Primary Care Physician (MD/DO, FM/IM) – Summary
- Provide comprehensive outpatient primary care services for a senior patient population with multiple chronic conditions
- Collaborate with a multidisciplinary care team in a value-based care model
- Deliver care in a modern, patient-centric clinic environment designed to foster wellness and community
Duties & Responsibilities
- Assess, diagnose, and manage patients with complex health issues such as heart disease, diabetes, and hypertension
- Maintain a patient panel with an average of 10-12 patients per day and a maximum of 650 patients
- Deliver high-quality care aligned with HEDIS metrics and value-based care goals
- Participate in patient satisfaction and quality improvement initiatives
- Supervise and collaborate with your dedicated care team, including medical assistants, schedulers, and referral specialists
- Perform routine office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Participate in an equitable call schedule based on the number of providers in the market
- Document patient encounters and care plans accurately and promptly in the electronic medical record

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Unrestricted and active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is a plus but not required
- Commitment to value-based care and patient-centered service
