Primary Care Physician (MD/DO, FM/IM) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Phoenix, AZ. The clinic is designed to provide holistic, respectful, and continuous care, especially for seniors managing multiple complex conditions.
Primary Responsibilities
- Provide comprehensive outpatient primary care to an adult and senior population, focusing on patients with complex chronic diseases such as heart disease, diabetes, and hypertension.
- Practice within a value-based care model, emphasizing quality, patient satisfaction, and improved health outcomes.
- Maintain a manageable patient panel, with an average of 10-12 patients per day and a maximum panel size of 650.
- Collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and receive full support from clinic leadership.
- Utilize proficient office procedure skills, including suturing, joint injections, skin biopsies, and basic wound care, as part of routine patient management.
- Participate in a rotating call schedule based on the number of providers in the market, ensuring balanced work-life integration.
- Document and manage care in accordance with regulatory standards, including HEDIS compliance and protocols for reducing hospital admissions and sick days.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active license to practice medicine in Arizona.
- Current DEA registration.
- Fellowship training in Geriatrics is a plus but not required.
- Strong proficiency in office-based procedures relevant to primary care.

Work Hours and Environment
