Primary Care Physician (MD/DO, FM/IM) – Full-Time

Primary Care Physician (MD/DO, FM/IM) – Summary
- Provide comprehensive outpatient primary care to a panel of adult and senior patients with multiple chronic conditions.
- Deliver patient-centered care with an emphasis on quality, satisfaction, and improved health outcomes.
- Work collaboratively with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists.
- Participate in a value-based care model that rewards excellence in care and patient engagement.
- Contribute to a positive clinic culture where patient respect and professional development are prioritized.
Duties & Responsibilities
- Manage a patient panel (maximum of 650 patients) focusing on seniors with 3-5 complex chronic diseases such as heart disease, diabetes, and hypertension.
- Conduct an average of 10-12 outpatient visits per day, ensuring each patient receives personalized, attentive care.
- Utilize electronic health records to document patient encounters, manage care plans, and coordinate with the care team.
- Lead and participate in interdisciplinary team meetings to coordinate patient care and improve care delivery.
- Perform in-office procedures including suturing, joint injections, skin biopsies, and basic wound care.
- Participate in a rotating call schedule, proportionate to the number of physicians in the group, to ensure continuous patient care.
- Ensure compliance with HEDIS measures, contribute to reducing hospitalizations, and uphold the company’s core values.
- Engage in patient education, chronic disease management, preventive care, and wellness promotion activities.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted license to practice medicine.
- Current DEA registration.
- Proficiency in office-based medical procedures such as suturing, joint injections, skin biopsies, and wound care.
- Fellowship training in Geriatrics is a plus, though not required.
