As a Primary Care Physician, you will work Monday through Friday in a collaborative outpatient setting, providing care for a senior patient population coping with multiple chronic conditions. With a manageable panel size and daily patient volume, you can devote the time and attention required for thorough patient care, preventative medicine, and strong patient-provider relationships. You will also participate in a call schedule designed to maintain work-life balance, coordinated with other providers in the market.

Primary Care Physician (MD/DO, FM/IM) – Summary

Duties & Responsibilities
- Provide comprehensive primary care for a senior patient panel (maximum 650 patients)
- Conduct patient visits (average 10-12 per day) focusing on prevention, chronic disease management, and patient education
- Work collaboratively with medical assistants, schedulers, referral specialists, and leadership
- Participate in a shared call rotation adapted to the number of providers in the market
- Perform common in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Promote patient engagement through continuity and personalized care
- Ensure compliance with HEDIS measures and company care standards
- Support wellness initiatives and activities in the clinic

Qualifications & Requirements
- Medical degree (MD or DO) required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures (suturing, joint injections, skin biopsies, wound care)
- Fellowship in Geriatrics is a plus but not required
