Primary Care Physician (MD/DO, FM/IM) – Jacksonville, FL – Full-Time OverviewJoin a forward-thinking healthcare organization as a Primary Care Physician (MD/DO, FM/IM) in Jacksonville, FL. This full-time opportunity is ideal for board-certified or board-eligible physicians dedicated to providing high-quality, compassionate primary care to an adult and senior patient population. Become part of a patient-centered medical home model in a modern outpatient clinic setting that emphasizes wellness, continuity of care, and community engagement.
Responsibilities- Deliver comprehensive primary care in an outpatient setting, focusing on adult and senior patients, including those with multiple complex chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a patient panel of up to 650 individuals, with an average of 10–12 patients per day, ensuring each patient receives personalized, attentive care.
- Work collaboratively within a value-based care model, prioritizing patient outcomes, satisfaction, and quality metrics.
- Participate in a supportive team environment, which includes a dedicated Medical Assistant, scheduler, referral specialist, and access to leadership support.
- Maintain compliance with HEDIS measures and contribute to the reduction of hospitalizations and emergency care for your patients.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care as clinically indicated.
- Participate in a shared call schedule, with frequency dependent on the total number of providers in the market (e.g., 1:32 weeks if 32 physicians).
- Engage in ongoing professional development and contribute to the positive culture of the clinic by embodying the company’s values.
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Proficiency in basic office procedures and electronic health record (EHR) systems.
