Primary Care Physician (MD/DO, FM/IM) – Summary
- Deliver compassionate, patient-centered outpatient primary care to a senior patient population with complex medical needs.
- Work within a value-based care model to improve patient outcomes and satisfaction.
- Join a collaborative care team with full support from leadership and staff.
- Practice in a newly established, accessible neighborhood clinic designed for patient comfort and engagement.
Duties & Responsibilities
- Conduct comprehensive outpatient primary care visits for an average of 10-12 patients per day.
- Manage a panel size of up to 650 patients, focusing on seniors with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Emphasize continuity of care and build long-term relationships with patients and their families.
- Participate in a Monday-Friday, 8 a.m. to 5 p.m. schedule, with call rotation based on provider team size.
- Utilize clinical skills in office procedures, including suturing, joint injections, skin biopsies, and basic wound care as needed.
- Work closely with a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support.
- Engage in performance metrics such as patient satisfaction, adherence to company values, compliance with HEDIS measures, and reduction in hospital admissions.
- Contribute to a positive clinic environment, supporting patient wellness through educational and community activities.

- Competitive base salary with total compensation potential exceeding $300,000, including performance-based bonuses.

Qualifications & Requirements
- MD or DO degree from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship in Geriatrics is a plus, but not required.
- Active, unrestricted license to practice medicine.
- Current DEA registration.
- Demonstrated proficiency in office procedures such as suturing, joint injections, skin biopsies, and wound care.
