Primary Care Physician (MD/DO, FM/IM) – Full-time

Primary Care Physician (MD/DO, FM/IM) – Summary- Full-time position focused on outpatient primary care
- Serve a predominantly senior population with complex conditions
- Work in a modern, value-based care model emphasizing patient wellness and quality outcomes
- Provide comprehensive outpatient primary care to a panel of up to 650 patients
- Average 10-12 patient visits per day, ensuring quality time and attention for each patient
- Lead a dedicated care team including a medical assistant, scheduler, and referral specialist
- Participate in quality improvement and value-based initiatives
- Utilize clinical skills in office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Engage with patients and their families to promote health education and preventive care
- Maintain compliance with clinical guidelines and company standards
- Participate in rotating call schedule as determined by the number of providers
- Collaborate with leadership and clinical teams to enhance patient experience and outcomes
- Competitive base salary with total potential earnings exceeding $300,000
- Performance-based bonuses up to 20% of base salary, based on patient satisfaction, adherence to company values, compliance metrics, and reducing hospitalizations

- MD or DO degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Fellowship in Geriatrics is a plus but not required
- Unrestricted, active medical license
- Current DEA registration
- Proficiency in routine office procedures (suturing, joint injections, skin biopsies, basic wound care)
