Primary Care Physician (MD/DO, FM/IM) – Full-time – Jacksonville, FL
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Jacksonville, FL. As a valued member of our organization, you will play a pivotal role in delivering high-quality, patient-centered care in a comfortable and accessible outpatient clinic setting. Responsibilities
- Provide comprehensive outpatient primary care services to a senior patient population, focusing on individuals with complex chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care within a value-based care model, emphasizing patient outcomes, satisfaction, and continuity of care.
- Maintain a manageable patient panel size, with a maximum of 650 patients and an average of 10-12 patients seen per day.
- Collaborate with an interdisciplinary care team, including medical assistants, schedulers, referral specialists, and leadership for optimal support.
- Participate in a shared call schedule, adjusted according to the total number of physicians in the market.
- Promote patient engagement and wellness through participation in clinic-based programs, such as exercise and art activities for seniors.
- Utilize proficient clinical skills in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Commit to ongoing professional development and quality improvement initiatives aligned with organizational goals.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Current, unrestricted, and active license to practice medicine in the state of Florida.
- Current DEA registration.
- Fellowship training in Geriatrics is a plus but not required.
- Demonstrated proficiency in outpatient office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
