Primary Care Physician (MD/DO, FM/IM) - Summary
- Full-time, outpatient primary care position serving a senior patient population
- Utilize a value-based care model to improve patient outcomes and reduce hospitalizations
- Work at a modern, patient-centered clinic designed to foster community and wellness
- Collaborate with a dedicated care team and experienced leadership
Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 650 patients, averaging 10-12 patients per day
- Focus on the management of complex chronic conditions such as heart disease, diabetes, and hypertension among senior patients
- Deliver care in accordance with value-based healthcare principles, including maintaining high HEDIS compliance and focusing on patient satisfaction
- Participate in a collaborative, team-based care environment, working closely with medical assistants, referral specialists, and support staff
- Utilize digital health records and technology to document patient information efficiently
- Perform minor office procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Engage in ongoing professional development and contribute to a positive clinic culture
- Participate in a manageable call schedule, proportionate to the number of providers in the practice
Salary & Benefits
- Competitive base salary of $260,000 with total earning potential up to $312,000, including performance bonuses based on patient satisfaction, company values, HEDIS compliance, and hospital utilization reduction

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certification or board eligibility in Family Medicine (FM) or Internal Medicine (IM) is required
- Fellowship training in Geriatrics is a plus, but not required
- Active, unrestricted medical license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and wound care
- Strong computer skills and experience with electronic health records
