Primary Care Physician (MD/DO, FM/IM) OverviewWe are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) to join our innovative healthcare team in Milwaukee, WI. Responsibilities- Provide comprehensive primary care services to a panel of senior patients, focusing on individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Work in an outpatient clinic setting with Monday through Friday hours from 8:00 a.m. to 5:00 p.m., ensuring patients receive consistent and attentive care.
- Maintain a patient panel of up to 650 individuals, with an average of 10-12 patient visits per day, allowing for quality interactions and continuity of care.
- Collaborate with a dedicated care team including a medical assistant, scheduler, referral specialist, and receive full support from clinic leadership to optimize patient outcomes.
- Participate in a call schedule shared among providers in the market, ensuring work-life balance.
- Utilize a value-based care model, prioritizing patient satisfaction, health outcomes, HEDIS compliance, and reduction of hospitalizations.
- Engage with patients in a culturally sensitive and respectful manner, supporting the overall mission of improving community health.
- Perform essential office procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) required.
- Fellowship training in Geriatrics is a plus, but not mandatory.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficient in office-based procedures, including minor suturing, joint injections, skin biopsies, and basic wound management.
