The new Primary Care Physician (MD/DO, FM/IM) role offers a Monday through Friday schedule, with manageable patient volumes to allow for thorough, attentive appointments. Primary Care Physician (MD/DO, FM/IM) – Summary- Full-time outpatient position dedicated to providing primary care to seniors with complex health needs
- Patient-centered approach, emphasizing continuity of care and compassionate service

- Provide comprehensive primary care to a panel of seniors with chronic and complex conditions
- Work collaboratively with an interdisciplinary care team

- Perform office procedures such as suturing, joint injections, skin biopsies, and wound care
- Maintain detailed and accurate patient records
- Participate in patient education and wellness initiatives
- Contribute to organizational quality benchmarks, including HEDIS compliance and patient satisfaction metrics
- Participate in an equitable call schedule, as determined by the care team size

- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license
- Current DEA registration
- Proficiency in office-based procedures
- Fellowship in Geriatrics is advantageous but not required
- Demonstrates a patient-first philosophy and strong commitment to quality care
- Experience or interest in managing care for seniors with complex health needs
- Excellent interpersonal and communication skills
