Primary Care Physician (MD/DO, FM/IM) OverviewWe are seeking a dedicated and compassionate Primary Care Physician to join our innovative healthcare team in Milwaukee, WI. This full-time opportunity is ideal for a board certified or board eligible Family Medicine (FM) or Internal Medicine (IM) physician who is passionate about delivering high-quality, value-based outpatient care to a senior patient population. Responsibilities- Provide comprehensive primary care services to an outpatient panel of senior patients, primarily managing individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care within the framework of a value-based care model, focusing on quality outcomes, patient satisfaction, and adherence to established clinical guidelines.
- Maintain a manageable patient schedule, with an average of 10-12 patients per day and a maximum panel size of 650, ensuring each patient receives personalized attention and continuity of care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care, streamline office procedures, and support positive patient outcomes.
- Utilize proficient PC skills to perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Participate in call coverage, with frequency determined by the total number of providers in the market (for example, 1:32 weeks if 32 physicians are present).
- Support the mission of creating a welcoming, respectful environment where patients and families feel valued and cared for.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active medical license to practice medicine in the state of Wisconsin.
- Current DEA registration.
- Proficiency in minor office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
