As a Primary Care Physician (MD/DO, FM/IM), you will manage a panel of seniors with complex medical conditions such as heart disease, diabetes, and hypertension. You will work alongside a dedicated care team—including medical assistants, schedulers, referral specialists, and strong leadership support—to deliver comprehensive care in a collaborative setting. The average daily patient load is designed to allow for high-quality, individualized care, and you will have ample support to ensure each patient's needs are met effectively.

We are seeking candidates who are passionate about primary care and value-based medicine, with board certification or eligibility in Family Medicine or Internal Medicine. While a fellowship in Geriatrics is a plus, it is not required. Candidates must hold a current, unrestricted medical license and active DEA registration. Proficiency in office procedures such as suturing, joint injections, skin biopsies, and basic wound care is essential.

Primary Care Physician (MD/DO, FM/IM) - Summary- Full-time, outpatient primary care position focused on seniors with complex health conditions
- Innovative care delivery model in a patient-centered clinic environment

- Provide comprehensive primary care to a panel of up to 650 seniors
- Diagnose and manage chronic diseases such as heart disease, diabetes, and hypertension
- Perform office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Collaborate with a dedicated care team (MA, scheduler, referral specialist) for optimal patient outcomes
- Participate in a value-based care model focused on quality, satisfaction, and reduced hospitalizations
- Maintain high standards of patient satisfaction, adherence to company values, and compliance with HEDIS measures
- Engage in ongoing professional development and participate in community wellness initiatives
- Work Monday to Friday, 8 am to 5 pm, with a call schedule proportional to the number of providers in the network
- Competitive base salary with total potential earnings exceeding $320,000
- Performance bonuses based on patient satisfaction, company values, quality measures, and reduced hospitalizations
