As a Primary Care Physician (MD/DO, FM/IM), you will play an integral role in promoting preventive care, managing chronic conditions, and improving overall patient outcomes. You will work alongside a dedicated care team, including medical assistants, schedulers, referral specialists, and supportive leadership. This integrated team approach allows you to focus on delivering high-quality, personalized care. Our clinics offer patients additional services, such as reliable transportation and on-site wellness activities, to remove barriers to care and foster a holistic approach to health.
This is a full-time opportunity, Monday through Friday, 8 a.m. to 5 p.m., with a patient panel size that maximizes meaningful interaction and continuity. Primary Care Physician (MD/DO, FM/IM) – Summary- Full-time, outpatient Primary Care Physician role focused on a senior patient population
- Value-based care model centered on patient outcomes, satisfaction, and wellness

- Clinic opening in May 2025 in Winston-Salem, NC
- Deliver high-quality, evidence-based primary care to seniors, primarily managing patients with complex chronic diseases such as heart disease, diabetes, and hypertension
- Maintain a manageable daily patient load (10-12 per day) to ensure thorough and personalized care
- Oversee a patient panel of up to 650 individuals, with a focus on continuity of care
- Lead and collaborate with a dedicated care team, including medical assistants, schedulers, and referral specialists
- Participate in value-based initiatives, including adherence to HEDIS measures and reduction of hospital admissions
- Utilize and support patient transportation services, and encourage engagement in clinic-based wellness activities
- Maintain accurate, up-to-date patient records and documentation in compliance with organizational and regulatory standards
- Participate in a shared call schedule (e.g., 1:32 weeks depending on provider count)
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed
- Competitive base salary with first-year earnings potential exceeding $310,000
- Annual bonus opportunities up to 20% of base salary, based on patient satisfaction, value alignment, and quality outcomes

- Medical degree (MD or DO)
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) – required
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in basic office procedures, including suturing, joint injections, skin biopsies, and wound care
- Fellowship training in Geriatrics is a plus but not required
- Demonstrates a passion for senior care and a commitment to improving patient outcomes
