Primary Care Physician (MD/DO, FM/IM) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) to join an innovative healthcare provider team in Winston-Salem, NC. This full-time outpatient opportunity is ideal for physicians who are passionate about delivering high-quality, value-based care to a senior patient population with complex medical needs. Our organization is committed to fostering a patient-centered environment, where individuals receive personalized attention and support from a collaborative care team. The new center is set to open in May 2025, offering a dynamic and supportive setting for both providers and patients.
Responsibilities
- Deliver comprehensive, patient-centered primary care in an outpatient clinic setting.
- Manage a panel of up to 650 senior patients, primarily individuals with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Maintain a manageable patient schedule, averaging 10-12 patients per day, to ensure quality care and attention to each individual.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, referral specialist, and receive full support from clinical leadership.
- Utilize a value-based care model, focusing on patient satisfaction, adherence to evidence-based guidelines (such as HEDIS compliance), and reduction of unnecessary hospitalizations.
- Participate in a shared call schedule, determined by the number of physicians in the market (currently, call rotation is approximately 1:32 weeks).
- Perform basic in-office procedures as needed, including suturing, joint injections, skin biopsies, and wound care.
- Engage in ongoing professional development and uphold the organization’s mission of improving the health and wellness of the community.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM); Geriatrics fellowship is a plus but not required.
- Current, unrestricted, and active license to practice medicine in the state of North Carolina.
- Valid DEA registration.
- Proficient in office-based procedures relevant to primary care.
