Primary Care Physician (MD/DO, FM/IM) Position Overview
We are seeking a dedicated Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Phoenix, AZ. Responsibilities
- Provide comprehensive primary care services to an outpatient population, focusing on seniors with chronic and complex medical conditions, including heart disease, diabetes, and hypertension.
- Utilize a value-based care model to promote patient wellness, emphasizing preventive care and reducing hospitalizations.
- Manage a panel of up to 650 patients, averaging 10-12 patient visits per day, to ensure continuity and quality of care.
- Collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and leadership support, to deliver coordinated patient care.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Maintain accurate, timely, and thorough documentation in compliance with regulatory and organizational standards.
- Participate in a call schedule based on the number of providers in the market, ensuring shared responsibility and work-life balance.
- Demonstrate the organization’s core values and contribute to a supportive, patient-centered clinic environment.
- Adhere to HEDIS compliance measures and contribute to achieving high patient satisfaction ratings.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Unrestricted, active medical license to practice medicine in the state of Arizona.
- Current and valid DEA registration.
- Proficiency in office-based procedures relevant to primary care.
- Fellowship training in Geriatrics is a plus but not required.
Employment Type
This is a Full-time position, with clinic hours Monday through Friday, 8:00 a.m. to 5:00 p.m. Call responsibilities are distributed equitably among market providers.
