As a Primary Care Physician (MD/DO, FM/IM), you will play a pivotal role in delivering holistic primary care within our outpatient setting. You will be responsible for managing a panel of senior patients, typically with three to five complex conditions such as heart disease, diabetes, and hypertension. Our model emphasizes quality over quantity, enabling you to focus on deeper patient relationships and better health outcomes.
You will be supported by a dedicated care team, including a Medical Assistant, scheduler, referral specialist, and comprehensive support from clinic leadership. This structure allows you to concentrate on clinical excellence while ensuring your administrative tasks are efficiently managed.

Duties & Responsibilities
- Provide primary care services to a panel of up to 650 senior patients
- Manage patients with multiple chronic and complex conditions, such as heart disease, diabetes, and hypertension
- Conduct thorough assessments, develop personalized care plans, and monitor progress toward health goals
- Participate in value-based care initiatives, focusing on patient satisfaction, HEDIS compliance, and reducing hospital admissions
- Collaborate closely with your dedicated care team to coordinate services and referrals
- Perform office procedures including suturing, joint injections, skin biopsies, and basic wound care
- Maintain accurate, timely, and compliant medical documentation
- Participate in rotating call coverage, with frequency based on the number of providers in the market
- Engage in wellness and educational activities within the clinic to support patient health
- Uphold organizational values, demonstrating respect and empathy in all patient and team interactions

- Competitive base salary with total earning potential up to $324,000

Qualifications & Requirements
- MD or DO degree required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted medical license and current DEA registration
- Experience with office-based procedures such as suturing, joint injections, skin biopsies, and wound care
- Proficiency with electronic medical records and basic PC skills
- Fellowship in Geriatrics is a plus but not required
