As a Primary Care Physician (MD/DO, FM/IM), you will manage a panel of seniors, typically with 3-5 chronic conditions such as heart disease, diabetes, and hypertension. Our value-based care model emphasizes quality, patient satisfaction, and preventive care, ensuring your clinical efforts lead to meaningful improvements in patient outcomes. The average daily patient load is intentionally kept low, allowing you to deliver thorough and attentive care to each individual. You will be supported by a dedicated care team, including a medical assistant, scheduler, referral specialist, and leadership support to ensure seamless patient management and optimal workflow.

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 650 senior patients with multiple chronic conditions.
- Provide care in a value-based model, emphasizing patient satisfaction, HEDIS compliance, and reduction of hospitalizations.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, and referral specialists.
- Utilize best practices in preventive care, chronic disease management, and wellness promotion.
- Perform office procedures as appropriate (e.g., suturing, joint injections, skin biopsies, basic wound care).
- Participate in a call schedule shared among market physicians; frequency based on number of providers.
- Document patient encounters accurately and timely using electronic medical records.
- Engage in quality improvement initiatives and live the organization’s values.
Salary & Benefits
- Competitive base salary of $260,000 with total earning potential exceeding $312,000, including up to 20% of base in performance bonuses.
- Bonus criteria include patient satisfaction, adherence to company values, HEDIS compliance, and reductions in hospital sick days.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship in Geriatrics is a plus but not required.
- Active, unrestricted medical license to practice medicine.
- Current DEA registration.
- Proficiency in office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
