Primary Care Physician (MD/DO, FM/IM) Job Description
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO, FM/IM) for a full-time opportunity in Milwaukee, WI. This role focuses on delivering comprehensive outpatient care within a value-based care model, serving a diverse population of senior patients with complex health needs. Responsibilities
- Provide high-quality primary care services to an outpatient panel of senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Manage a panel size of up to 650 patients, with an average daily patient census of 10-12, ensuring personalized attention and continuity of care.
- Collaborate with a dedicated care team, including Medical Assistants, schedulers, referral specialists, and leadership support, to deliver coordinated and efficient healthcare services.
- Utilize a value-based care approach to improve patient satisfaction, comply with HEDIS measures, and reduce hospital admissions and sick days.
- Participate in a rotating call schedule based on the number of physicians in the market (e.g., 1:32 weeks if 32 physicians are present).
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as indicated.
- Embody and promote the organization’s core values in every patient interaction and across the clinical team.
- Support patient engagement by encouraging participation in wellness activities, such as exercise and art programs offered at no cost to patients.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) is required.
- Current, unrestricted, and active medical license to practice in the state of Wisconsin.
- Current DEA registration.
- Proficiency in performing essential office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus but not required.
