Primary Care Physician (MD/DO, FM/IM) - Summary
- Serve as a Primary Care Physician in a progressive, patient-first outpatient clinic environment
- Work closely with a supportive care team, including medical assistants, schedulers, referral specialists, and leadership
- Focus on building long-term relationships with patients, providing high-quality, individualized care

Duties & Responsibilities
- Provide comprehensive primary care to an adult and senior population, primarily consisting of individuals with 3-5 complex chronic conditions such as heart disease, diabetes, and hypertension
- Deliver patient-centered outpatient medical care, including preventive, acute, and chronic disease management
- Lead and coordinate a small panel (maximum 650 patients) to ensure continuity and consistency in care
- Maintain an average of 10-12 patient visits per day, enabling thorough, quality interactions
- Collaborate with an integrated care team for optimal patient management and efficient workflow
- Utilize electronic health records and maintain accurate, timely medical documentation
- Participate in value-based care initiatives, focusing on improving patient satisfaction, HEDIS compliance, and reducing hospital admissions
- Engage in regular communication with patients, families, and caregivers, fostering strong provider-patient relationships
- Practice evidence-based medicine while adhering to organizational protocols and quality standards
- Participate in call rotation as needed, based on the number of providers in the market
- Perform minor office procedures such as suturing, joint injections, skin biopsies, and basic wound care as clinically indicated
- Promote healthy lifestyle activities and wellness initiatives offered by the practice, including exercise and art activities for seniors
- Ensure compliance with all applicable legal, ethical, and regulatory guidelines

- Competitive base salary, with total earning potential exceeding $300,000 annually, including performance-based bonuses

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM) required
- Fellowship training in Geriatrics is a plus but not required
- Current, unrestricted medical license and active DEA registration
- Proficiency in minor office procedures, including suturing, joint injections, skin biopsies, and basic wound care
- Commitment to patient-centered, evidence-based medical practice
- Strong communication, organizational, and teamwork skills
- Familiarity with electronic health record systems
